Program Eligibility

The patient needs specialty consultation/care OR the patient has a chronic illness. Specialty consultation is defined as a service that the primary care physician can not provide.
Patient income (using the Basic Health Form) is 200% or less of the Federal Poverty Level.
The patient is NOT currently covered by any other healthcare coverage/program.
Patient has applied for Community Health Plan of Washington BHP/Group Health programs and/or Medicaid programs.
The patient can provide a Thurston County physical and mailing address and phone numbers for us to contact them.
The patient can provide one (1) document to prove residency in Thurston County that has their current address. Valid documentation includes:
Lease, rental agreement, mortgage with their name on it.
Utility and other "main" bills (ex: auto/home insurance, etc.)
If living with another, a letter from the owner/lessee giving address, length of time patient has lived there and anticipates living there in the future.
The patient can provide one (1) valid identification including:
Washington state driver's license
Washington state ID card
